2010-2011 GK-12 Transforming Experiences
Resident Application Form

Application and ALL additional documents must be received by 5:00 p.m. on Friday, January 29, 2010.

Notification of awards will be made in early March 2010.

Please note:

By submitting this form, I hereby apply for consideration to GK-12 Transforming Experiences Project at the University of Colorado Denver, and certify that to the best of my knowledge all of the below statements are true and complete. If accepted and I agree to participate in the program from July 1, 2010 to June 30, 2011, I will comply with all applicable UCD and NSF Program terms and conditions governing my participation. I authorize the GK-12 Program personnel to access my UCD student records and monitor my academic progress. Additionally, I agree to abide by all University policies and will commit to complete all expectations as outlined on the Applicant Information page.  After my participation as a GK-12 Resident, I agree to maintain contact with the Project’s personnel in order to track my career development and evaluate the outcomes of the GK-12 project. 
 

I understand that, as a GK-12 Resident, I will commit to:
         -Completion of all summer project requirements:
                    -Participation in the GK-12 Resident Workshop, July 12th-16th, 2010
                    -All participant orientation, July 26th-30th, 2010
         -Successful background check conducted through the Colorado State Police and reported to the assigned middle school
         -Participate in monthly Resident seminars and meetings
         -Submission of weekly & monthly time and activity log
         -Spend time in the middle school on enrichment/ development activities and on preparation and reporting activities (15 hours per week average)
         -Participate in STEMapalooza in the fall of 2010
         -Work in partnership with GK-12 Lead teachers on classroom multi-disciplinary & inquiry-based lessons
         -Teach a multi-disciplinary & inquiry-based lesson(s) or unit(s)
         -Present papers and research at (at least) one conference
         -Participate in ongoing project evaluation and research
         -Respond in a timely manner to e-mail messages from GK-12 personnel
         -Not holding any job outside of the GK-12 Project
         -Maintain status of full-time student
