1. Fill out the form below (3 Sections). 

2. Click "Submit Form" at bottom of page. (You will receive a confirmation email).

3. Make your donation one of the following methods:

                      - Credit/debit card during player registration (if you are registering a child)

                                                                     OR

NOTE: UNIFORM AND FIELD BANNER SPONSORS WILL RECEIVE AN ELEGANT SPONSOR PLAQUE, HAVE COMPANY LOGO DISPLAYED ON MLL WEBSITE WITH LINK TAKING VISITORS DIRECTLY TO BUSINESS WEBSITE AND ARE INVITED TO PARTICIPATE IN OPENING DAY PARADE EVENTS (WEATHER PERMITTING) FOR THE OPPORTUNITY TO INTERACT WITH MEMBERS OF COMMUNITY AND PROMOTE BUSINESS.
