The information provided on this form will be used to prepare your enrollment certification/verification. To process your certification, all fields must be completed and submitted to the Veteran & Military Services office once you register for classes. You are required to report any subsequent changes to your enrollment (e.g. number of credits, withdrawals, additions, etc.) to the Veteran & Military Services office. If Veteran & Military Services does not have your certificate of eligibility (letter from the VA) for VA benefits on file, you will not be certified for any classes, unless the Veteran Coordinator approves it in writing.
