CGCE Course Withdrawal Form - Undergraduate
This course withdrawal form is for CGCE students only.
Day Division students should contact the Office of the Registrar for course withdrawal forms.
Withdrawing from a course can affect a student’s class standing, financial aid and course of study, so before you withdraw from a course, consider the following:
Academic Plan: This form requires the professor’s and your advisor’s signature. It is your responsibility to contact your course instructor and academic advisor. This is designed to inform the professor of your intent and action to leave the course and to discuss the impact of the withdrawal with the faculty advisor. An acknowledgement from the professor and advisor via email is permitted.
Financial Aid/Veterans: If you receive financial aid, and need to maintain satisfactory academic progress for continued financial aid eligibility, please contact Financial Aid, at firstname.lastname@example.org or 413-572-5218. For all Federal Direct Loans, and most other forms of financial aid, you must be enrolled at least half-time. For undergraduate students, half-time is defined as 6 credit hours.

Refunds & Transcript Notation: Refunds of tuition and fees of credit courses, as well as transcript notation, if any, are made on the basis of the date and time of receipt of a student’s withdrawal. Please refer to the Withdrawal and Refund from Courses under Policies for Graduate & Continuing Education for the full schedule. Students will receive a grade of ‘W’ on their academic transcript for this course. A ‘W’ is a non-punitive grade that is not used in calculating a student’s quality point average.
Withdraw Deadline: The official withdrawal dates are published in the Continuing Education Catalog.
