Graduate Certificate Application

Application Instructions:

·        Complete below application and submit $50 application fee. (Fee waived for veterans). (To submit application fee, please contact the Graduate and Continuing Education office.)

·        Submit official, sealed transcripts from all colleges and or universities previously attended. Opened transcripts are not accepted.

·        Two letters of recommendation- either uploaded to this application or email to wsucgce@westfield.ma.edu.
