| This Application is for volunteers who are looking to come and serve at Salem Acres for 2 weeks or less. |

| We are excited that you would like to partner with us in ministry in 2014! In order to ensure that you are well-prepared for your ministry at Salem Acres, all Staff and Volunteers must participate in at least one On-Site Staff Training Weekend at Salem Acres (choose your date below). In addition, please download your reference forms from our website before you continue and request a police check (if you are over 18 years of age) and have not submitted one to Salem Acres in the past 3 years. |

| Please have a church elder or pastor download the reference form from our website and fill it out. Once completed, please email them to the volunteer coordinator at least two weeks prior to your On-Site Staff Training Weekend. If you are older than 18 years, and have not submitted a Police Check in the last three years, please contact the volunteer coordinator and have a signed "Criminal Record Check" letter sent to you. This letter is to be taken to your local Police Department to obtain a Criminal Record Check. Police Checks can take up to 3 months to process, so please request them early. |

| NEW POLICY:
We will no longer be stocking medication for minors. Please provide any medications (including Tylenol, Ibuprofen, etc) for your child that you anticipate they may need while at camp in a resealable bag with the Child's Name, Parent/Guardian Name and Emergency Phone Number.
All medications in this bag must be in the original packaging with appropriate dosage for their age/size.
Dosage other than what is recommended on the packaging must be indicated in writing, signed by the parent/guardian and included in the bag.
We are no longer able to administer ANY medication to your child that is not in this bag. |
| If any medication is taken on a regular or semi-regular basis, the parent/guardian is responsible for ensuring that the appropriate medication is sent with the volunteer. ALL medication is to be given to the camp nurse with written instructions upon arrival at camp. Medication of any kind is NOT permitted to stay with the volunteer while they are at Salem Acres. All medication should be picked up from the camp nurse by a parent/guardian on the last day of camp when you come to collect your child. |
