ABOUT THE ONLINE SURVEY

Online submissions CANNOT BE SAVED once begun so be prepared to complete your submission before beginning.  If you have long text responses you may wish to upload responses as a survey attachment to avoid losing any responses should your browser unexpectedly close.

All submissions are secured with 128-bit encryption GeoTrust SSL certificates.

WHEN

Rolling Deadline for Fleet Ratings: Submissions can be made at any time.  Once you have received confirmation of a complete submission your fleet will receive its one, two, or three leaf rating within 3 weeks.

Award Deadline: Submissions received before January 16, 2015 are eligible to receive a Green Fleets of the Bluegrass Award at the spring KCFC member meeting.  

WHAT

Data on the following areas will be collected: Contact Information, Data Year, Vehicles & Fuel, Operation, Maintenance, Partnership, Strategy, and Transparency.   For fleets operating in multiple states, only submit data related to operation in Kentucky.

TWO SUBMISSIONS are required:1) Baseline data AND 2) Year-end data.  Past participants need only to submit this year's data.

Be sure to detail improvements made in each area since your last submission so that your fleet can earn recognition in those areas. Credit will not be given if no improvements have been demonstrated.
