SCHOOL/INSTITUTION RESOURCES- Please provide details about your school/institution so that we may ascertain suitability for the NFSP Program.
OTHER RESOURCES- If available, provide additional facilities you have access to at your school; these may include tennis courts, sports fields, pools, additional gyms, equipment, etc.
Describe the availability of water resources with proximity to your school and how they might be utilized in conjunction with NFSP.
Do students currently have access to fishing equipment at school? 

EXPERIENCE: Describe any past experience the school has with fishing program(s):
Do any teachers involved in participating in NFSP have experience in fishing? If so, please describe their experience level and planned involvement.
NFSP CERTIFICATION - at least one person must be certified by an NFSP Educator at a Personal Training, or via the online self-study LMS, NFSP CAST, prior to teaching the NFSP curriculum.

CROSS CURRICULAR OPPORTUNITY - Will NFSP have cross-curricular involvement with school subject matter? If so, what will that look like with the overall school curriculum goals?
e.g., science, language, etc.

VIABILIITY/SUSTAINABILITY - Describe how your school might support the viability/sustainability of the NFSP program. School support may include administration, staff, teachers, parents and after school programs.
How long do you foresee the NFSP program remaining in your school's PE or other curriculum? What are some of your ideas for keeping the program viable for future years? Please relate this issue to funding, staffing, permanent program adoption into the school and community support.
