SELECT PACKAGE: Rates are per person - Packages include standard accommodations (number of nights varies depending on package chosen), three meals per day + gratuity for meals (dinner upon arrival, breakfast on departure), boot camp activities (5 hours of classes each full day), one fifty minute spa treatment + spa gratuity, one 30 minute health coaching session (phone or Skype) as well as all taxes.

PAYMENT & CANCELLATION INFORMATION ~ Complete the information below and agree to our policies. Once you submit this form, you will be redirected to complete your deposit payment. Your retreat balance payment, due 14 days prior to arrival, will be completed by you via secure invoice sent to the email address you have provided.
AUTHORIZATION: I authorize Vita Vie Retreat, Inc to charge my credit card for $750.00 as a deposit for my retreat. I understand that the balance of my retreat will be due 14 days prior to arrival. I understand that if I do not complete my balance payment 14 days prior to arrival that my registration is subject to cancellation and my deposit may be forfeited.	*	I agree to the payment policy.
AGREEMENT: I understand that I must give a cancellation notice of more than 14 days prior to my arrival date to receive a retreat credit in the amount of $750.00, equal to my deposit, to be used to reschedule my retreat within 1 year of my cancellation date. I understand that if I do not give at least a 14 day cancellation, my deposit is non-refundable and a retreat credit will NOT be issued. I understand that a No Show/No Call on my arrival day will result in being charged the full amount without the ability to reschedule or receive a refund.	*	I agree to the cancellation policy.
