Course Withdrawal Form - Graduate
Withdrawing from a course can affect a student’s class standing, financial aid and course of study, so before you withdraw from a course, consider the following:
Academic Plan: This form requires the professor’s signature. This is designed to inform the professor of your intent and action to leave the course. An acknowledgement from the professor via email is permitted. It is recommended you speak with your faculty advisor to discuss the impact of the withdrawal and your program study.
Financial Aid/Veterans: If you receive financial aid, and need to maintain satisfactory academic progress for continued financial aid eligibility, please contact Financial Aid, at firstname.lastname@example.org or 413-572-5218. If you receive Veterans benefits, please contact Veteran Affairs to ensure your benefits have not been affected, at email@example.com or 413-572-8370.
Refunds & Transcript Notation: Refunds of tuition and fees of credit courses, as well as transcript notation, if any, are made on the basis of the date and time of receipt of a student’s withdrawal. Please refer to the Withdrawal and Refund from courses under Policies for Graduate & Continuing Education for the full schedule. Students will receive a grade of ‘W’ on their academic transcript for this course. A ‘W’ is a non-punitive grade that is not used in calculating a student’s quality point average.
Withdraw Deadline: The official withdrawal dates are published in the Graduate and Continuing Education Catalog.
