By completing this form and clicking "Submit," I hereby release the organizers, supporters and volunteers of the Lee's Summit Photo Walk from liability for injury or loss occuring during the walk. I understand that although the walk was planned with safety in mind, I am responsible for the actions taken by myself and any minors who accompany me and for the risks that accompany those actions.
I agree to:
1. Keep minors accompanying me with me at all times.
2. Take reasonable precautions when crossing streets.
3. Watch for and avoid hazards such as uneven pavement, tree roots, insects, etc.
4. Stay off of private property unless I personally obtain the permission of the owner. This includes leaning or hanging on fences or gates and touching plants and animals.
5. Move out of the way to allow traffic to pass.
6. Notify walk organizers or leaders immediately if I (or minors accompanying me) begin to feel dizzy, weak, disoriented, overheated or am (are) injured.
7. Ask for permission to enter a business to take pictures.
Submitting this form is the same as placing my signature on the above statement.
