Assumption of Risk and Waiver for Liability for Icetown

IN CONSIDERATION of my being allowed to use the Icetown facility and related activities, I, the undersigned, hereby agree to and acknowledge the following:
1. I KNOWINGLY AND FREELY ASSUME ALL RISKS, both known and unknown, and assume full responsibility for my participation; and, agree not to participate while under the influence of drugs or alcohol; and,
2. I will comply will all rules and regulations. If I have any question or observe any unusual or unnecessary hazard during my participation, I will immediately bring such to the attention of the nearest Icetown employee. I agree not to participate while under the influence of drugs and alcohol; and,
3. I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, HEREBY RELEASE, INDEMNIFY AND HOLD HARMLESS, Icetown, its owners, affiliates, agents, and employees, and their successors and assigns ("Releases"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT INDUCEMENT.	*	Yes No
