2009 MS Parent Survey on Teachers

Teacher Survey 2009-10

Please fill out the following form on your child’s teacher giving prayerful thought to his/her strengths and areas of improvement. A listing of strengths as this teacher has worked with your child is important and reinforcing to the teacher’s efforts.

Your individual perspective on areas of improved effectiveness often helps the individual teacher focus plans for professional development.

This year, the Middle School is asking that these notes go directly to the MS Principal if the teacher has been here less than four years. The principal will then share these with the teacher. If the teacher has served for four or more years, the survey results go directly to the teacher without review by the Middle School Principal.

We know that as parents, you are not able to be in the classroom. However, we feel that you still have the ability to prayerfully list strengths and areas of needed improvement. As you fill this out, you might consider the following areas:

Interpersonal relationships
Spiritual and character development of your child
Classroom/home communication
Meeting your child’s academic needs
Overall instructional issues
Overall classroom learning environment

Please note that homework issues should be addressed on the homework tracking form.

This year, the Middle School is asking that these notes go directly to the MS Principal if the teacher has been here less than four years. The principal will then share these with the teacher. If the teacher has been at CDS for 3 years or more, the results of this survey will be sent directly to the teacher (with no review by the Middle School Principal) Please note that you are always welcome to share these comments with the teacher
