Instructions: Complete this form to sign-up for a Fall 2008 Structured Follow-Up (SFU) course with the Rocky Mountain Middle School Math and Science Partnership. This form ONLY signs you up for the Spring SFU courses. It doesn't sign you up for spring or summer semester content classes. All fields with a * are required fields. The form will not submit until you complete the required fields.

This form DOES NOT register you for college credit. You must register for college credit through the university SMART system.

SFU Course List for Fall 2008
All SFU courses occur on Saturdays on the following dates: 9/13, 10/4, 11/8, 12/6.
Science SFU Classes: BIOL 5001 Human Systems and Heredity, PHYS 5102: Light, Color, and Geometrical Optics.
Math SFU Classes: MATH 5001 Algebraic Patterns and Functions 1 (session 1 lesson study), MATH 5008 History of Math, MATH 5009: Math Modeling, MATH 5007: Discrete Math. NOTE: Algebra II sessions 1 and 2 are combined and will be taught in Spring 2009
