Summer 2009 CONTENT COURSE APPLICATION

*Tuition is $800 for the onine classes and $400 for the face to face classes.
*Stipends will be given to partner school district participants please see http://rmmsmsp.cudenver.edu/Stipend.html or contact Zula Ganbold at 303-556-2735 with questions.
*There are no corresponding SFUs for these courses.
INSTRUCTIONS: Complete this application form to apply to take a Summer 2009 Content course with the Rocky Mountain Middle School Math and Science Partnership. This form is ONLY an application to be considered for acceptance to take a Summer 2009 content course. Submission of the application form does not sign you up for the classes. We review all applications and place applicants into courses based on our selection criteria and the course preferences of the applicant. We will notify you by April 27, 2009 after we review all applications to tell you if you were or were not placed into a class.
All fields with a * are required fields. The form will not be submitted until you complete all required fields.
This form DOES NOT register you for college credit. You register for college credit through the University of Colorado Denver registration system at https://hydra.cusys.edu/pinnacle/cgi-bin/sisget.cgi/dn/awssgnsn/ after you have been notified by us that you were placed into a class.
Note: We will offer ten Summer 2009 content courses and participants are allowed to take two classes.
