Parent / Guardian Release - Hold Harmless Agreement:

I, the parent/guardian of the child named in this registration form, hereby give my permission for her participation in any and all activities of the volleyball league and or training. I do hereby waive, release, absolve, indemnify and forever discharge Coaches; it’s Directors, Staff, Club North Volleyball, KC Premiere Volleyball, and Faye & Don Alexander from all claims for damages, injuries, or loss to a person or property which may be sustained during participation in league activities or while at the training site whether or not damages, injury, or loss is due to negligence. I understand that participation in this league and or training program will require physical activities of a nature which could result in injury to participants. I further acknowledge that she is physically able to participate in all activities and hereby authorize her participation. I agree to allow my child to be treated by a certified athletic trainer or physician (if necessary), and to assume all costs related to such treatment.
